WAIVER FORM
for Use of Non-Program (Volunteer) Animal

The Utah Tech University recognize that at times it may be beneficial for students and other individuals or organizations to volunteer their personally owned animals to be used in class for educational purposes. Such purposes include but are not limited to demonstrations, skills practice, treatments, procedures, etc.

“Volunteer Animal” refers to any animal that is not an officially designated program animal.

“Owner” refers to the person responsible for the animal, i.e. its legal owner, guardian or delegated individual.

Policies of the Utah Tech University Programs

1. Instructors/Faculty at Utah Tech University will:
1. Protect the Volunteer Animal from physical and psychological harm to the best of their ability, but the Owner has the right to deny, interrupt or halt any procedure or demonstration at any time.
2. Protect the students public from any Volunteer Animal deemed unsafe or potentially harmful to the students and staff. University employees have the right to refuse treatment or use any Volunteer Animal at any time.
3. Use the Volunteer Animal for instructional or demonstration purposes only. IACUC approved SOPs and course protocols when appropriate will be utilized when working with Volunteer Animals.
4. Obtain verbal and/or written permission from the Owner to use the Volunteer Animal for procedures.
5. Provide supervision over any procedures being performed by a student or the public.
2. Utah Tech University and any of its faculty, staff and employees will not be held responsible for any costs or charges resulting from the use of Volunteer Animals while on campus, including but not limited to, veterinary expenses, medications, or animal-induced injuries to students, staff, or other Volunteer Animals.
3. University veterinarians or staff are not expected to render a diagnosis, prescribe medication or perform follow-up care on any Volunteer Animals. Should the Volunteer Animal become injured or experience a medical crisis during class, the Univeristy staff may administer first aid and will direct the Owner to immediately transport the animal to a local veterinary clinic for treatment at the Owner’s expense.

Owner Responsibilities

1. As a condition of participation, Owner must:
1. Transport the Volunteer Animal to campus in an approved carrier or cage (cats and small dogs) OR on a secure leash & collar/halter (large dogs). The owner is also responsible for timely pickup if dropped off (within one hour) and transport of the Volunteer Animal from campus once the lab exercise is concluded.
2. Show proof of current rabies vaccination status in pets that are required by city law.
3. Provide own food, bowls, bedding, litter boxes etc. for the Volunteer Animal during the stay on campus (if requested by the instructor).
4. Clean up after the animal, including any excrement, dirty cages, etc.
5. Immediately transport the Volunteer Animal to a local veterinary hospital should the pet become injured or require extended veterinary care. The Owner will assume financial responsibility for said veterinary care.
2. For and in consideration of his/her Volunteer Animal’s participation in the Utah Tech program, the Owner hereby releases and discharges Utah Tech University, its employees, agents, or representatives, from any and all causes of action for injury, illness, property damage, death or loss of any kind whatsoever occurring to him/herself or his/her Volunteer Animal, arising as a result of the use of his/her Volunteer Animal on campus. The Owner further agrees that under no circumstances will he/she or his/her heirs, executors, administrators, and/or assigns present any claim for personal injury, illness, property damage, death or loss of any kind whatsoever against Utah Tech University, its employees, agents, or representatives
